Registration Forms

The process to apply for registration at Bruntsfield Medical Practice is a 4-stage process.


If you haven’t already, you need to complete the pre-registration form to book a registration slot. We can only register so many people per week and these slots go fast. We only accept registration paperwork from those with an agreed slot.

After you’ve given us your details, you will be contacted and given the link to this page. Since you’re here we will assume you have completed stage 1 and are ready for stage 2.


For each person wanting to register with the Practice you need to complete the Registration Form


Upload the completed registration form to us using the form below (separate uploads for each patient in your household please), along with:

  • A form of identification (such as a passport/Residence Permit, a photo driving licence, or photo bus pass)
  • Proof of your address (such as a Council Tax or Utility bill)

While our preference is to receive the paperwork electronically, if you are unable then please send us this information either through the mail using the address at the bottom of the page, or by handing it in in person.

Our preference is to receive these forms electronically, so if you can please use the upload form above to send them back to us, but if not possible, please either hand them in at reception or mail them back to the address at the bottom of the page.
